Our referral process is designed to be simple, clear and accessible.

To begin services, please complete our Referral Form and send it to us via email. Once received, our team will contact you to discuss next steps.

Step 1:
Download the Referral Form

Download The Mindbuilders One Referral Form from this page.

The form can be completed by:

  • Participants or family members

  • Support Coordinators

  • Health or education professionals

  • Other referring agencies

Step 2:
Complete the Referral Form

Please complete all relevant sections of the form to the best of your knowledge.
Providing clear and accurate information helps us assess suitability and respond promptly.

Information typically includes:

  • Participant details

  • NDIS status and plan information (if applicable)

  • Reason for referral

  • Requested services

  • Current supports and any identified risks

Step 3:
Email the Completed Form

Once completed, email the Referral Form to:

Step 4:
We Will Contact You

After we receive the referral form:

  • Our intake team will review the information

  • We will contact the referrer or participant to discuss service options

  • Additional information may be requested if required

We aim to respond as soon as possible to confirm next steps.

What Happens After Referral?

Depending on the referral and service request, next steps may include:

  • An initial intake or consultation

  • Confirmation of NDIS funding suitability

  • Allocation to the appropriate clinical or support team

  • Scheduling of assessments or commencement of services

All referrals are reviewed in line with participant needs, capacity, and NDIS requirements.
